The Food and Drug Administration (FDA) regulates commercial infant formulas. These three types of formulas are the most popular:

Protein-based formulas made with cow milk. Most infant formula is made with cow’s milk that’s been altered to resemble breast milk. The formula is easier to digest and has the correct balance of nutrients. Most infants can tolerate cow’s breast milk. Other infant formula is available for some children who are sensitive to proteins from cow’s breast milk.

Soy-based formulas. It is possible to make your child’s meals vegetarian by using soy-based formulas. Soy-based infant formulas might also be an option for babies who are intolerant or allergic to cow’s milk formula or to lactose, a carbohydrate naturally found in cow’s milk. What Parents Should Do When Choosing European Baby Formula

In the US, there are over 50 baby formulas. You can select from organic or conventional soy milk. Parents also have the option to purchase formulas for colicky, fussy, and gassy babies. Baby formula is one the most closely regulated foods products in America. The Food and Drug Administration (FDA), regulates the nutrition and vitamins and sets strict guidelines about packaging and labeling.

However, parents are increasingly buying European formulas despite the fact that it is technically illegal to bring them into the US. Numerous Facebook groups dedicated to European formulations allow parents to share spreadsheets and details about the ingredients as well how they compare with their US counterparts. Some caregivers say they choose European brands because of the availability of certain options, such as those made from goat milk or from pasture-raised milk cows. These formula options aren’t available in the US FDA-regulated forms. European brands are perceived to be higher-quality and have stricter formula regulations.

The premium is paid by American parents who purchase European formulas. Holle Organic Formula (Germany) is available for purchase on several online stores that offer imported EU formulas. Anthony Porto (a Yale University pediatric gastroenterologist, as well as a professor of pediatrics), conducted an analysis to determine how European formulas differ from US formulas and whether they met FDA guidelines.

According to the authors of this study, there is a risk that these formulas could be imported. One reason is that instructions for certain European formulas can be difficult for English-speaking families to properly prepare them. The European formulas mix differently to US formulas. However, Porto’s research found that most European formulas meet the FDA nutritional guidelines. But it is not always so. Bridget Young is a pediatrician at the University of Rochester School of Medicine and Dentistry and has created a site with information on baby formula. For instance, certain sugars like corn syrup are banned by the EU in milk-based formulas. It also requires that at minimum 30% of carbohydrates be from lactose. Young and Porto agreed on the preference for formulas that have lactose in them. And it’s not difficult to find formulas in the US that meet the EU’s carbohydrate requirements (including the formulas we recommend in our guide The EU and US require most of the same vitamins and minerals in baby formula. Iron and DHA are some of the key differences. US formulas often contain greater amounts of iron than European counterparts. However, the American Academy of Pediatrics recommended that iron be maintained at a high level in order to reduce iron deficiency. By contrast, the EU recently began requiring DHA in much higher levels than are found in most formulas available in the US (the FDA does not require DHA at all, but most US formulas have it). While more DHA can be safe, Young noted that breastmilk contains DHA in a large amount. However, research has not supported the claims of higher levels found in European formulas.

Bobbie, the US-only infant formula that can meet EU DHA standards is at the moment. Bobbie, a formula that Bobbie describes as European-style, is being marketed to be an FDA-regulated substitute for European formulas. Bobbie’s iron levels are within the EU’s limits. And, like European formulas, Bobbie lacks many of the “extra” ingredients found in other US organic formulas: lutein, beta-carotene, taurine, nucleotides, and L-carnitine (it does have naturally occurring L-carnitine, as do all milk-based formulas). Although they can be found in breastmilk and have been shown to improve formula’s effectiveness, research is still lacking.

Some parents choose EU formulas because there are more milk-based options, including some made with whole milk. In the US, all milk-based formulas including Bobbie take away the milk fat and then add in plant-based fats, Young said. Although there are some potential benefits to using milk fat, A milk fat component known to be beneficial is the milk fat membrane (MFGM). Some of Enfamil’s premium formulas add MFGM because a few (industry-affiliated) studies have shown it provides a modest infection defense cognitive benefit for infants, for example. Young explained that infant formulas that are based solely on whole milk “will be receiving some of that natural dairy fat globule material.” Although the FDA has not banned formulas made from goat’s breast milk (no company is currently making one which meets FDA standards), Young noted that they can also use whole milk formulas. Caretakers are attracted to these formulas for the belief they are more “hypoallergenic,” which is an allergy or sensitivity to cow’s breast milk. Porto did say that infants with allergies to cow’s milk are rare and would probably react to goat’s milk.

European formulas have gentle options, which aren’t available in the US. A caregiver may request (or recommend) that a gentle formula be made with hydrolyzed proteins or partially broken down proteins. All of the US formulas contain corn sugar, either maltodextrin, corn syrup, and both. There are also partially hydrolyzed formulas in Europe that contain lactose, the only carbohydrate. Experts prefer these for babies. Parent’s Choice Enfamil Reguline is the US’s only partially hydrolyzed formula. It contains at least half of the lactose and has no intact protein.

European formulas have been the standard for formulas made with organic milk of grass-fed cows. Organic formulas are available (e.g. Earth’s Best Organic Infant Formula) and Similac Pure Bliss which is made of (non-organic), milk from grass-fed cattle. But Bobbie is currently the only US-based infant formula. It sources its milk both from organic and pasture raised cows. Bobbie, which costs $24 per 400g (and $1.60 for a 6-ounce container), is more expensive than many of our other recommendations.

Even though there are some differences between US formulas and European ones, the majority of these are very minor.
